How do I submit a report?
Safety reports can be submitted through the club billing system or via the Flight Safety Officer contact block on this page.
The required forms are available through the links below:

- Complete the occurrence form: Occurrence Form
Reporting occurrences is aimed at continuously improving flight safety.
The following list of reportable occurrences helps with that.
Reportable occurrences
- Bird or animal strike
- Loss of control of the aircraft
- AIRPROX
- Airspace infringement
- Technical defects that arise during flight operation
- Pilot incapacitation during flight
- Occurrences related to “I am safe” situations
- Problems during flight operation that affect flight safety
- Emergency landings and precautionary landings
- Runway or taxiway incursions and excursions
- Fire or smoke on board the aircraft
- Incorrect loading or loading outside the envelope
- Emergency calls, both PAN and Mayday
- Damage to the aircraft or damage to others caused by contact with the aircraft
- Getting lost
- Injuries to yourself or third parties
- Incorrect documentation, procedures or similar issues
- Oil or fuel problems
- Problems related to the condition of the airfield
- Occurrences not listed here that nevertheless affect safety
